Service Coordination

The Medicaid Service Coordinators (MSC) work with eligible individuals 3 years of age and older to identify unmet needs, desires and future goals to assist in creating a plan of action known as the Individualized Service Plan (ISP).   Strengths, capacities and ambitions are carefully considered along with the input of family members and guardians. The service coordinators reach out to area affiliates in securing the supports and services necessary to meet those needs. 

Eligibility for MSC is determined by The Office for People With Developmental Disabilities and the New York State Developmental Services Office (DSO). Individuals and their families may benefit from a variety of services to which the MSC provides linkage.  Some of the Upstate Cerebral Palsy programs and services available include: recreation, sports and leisure programs, vocational opportunities in the community, habilitation skills training, assistive technology, home environment modifications transition services from home living to independent community living through the WISH Program (Work-Independence-Support-Home), assistive technology and equipment, individualized habilitation services, outreach, advocacy, benefits assistance and legalized guardianship services. MSCs working for Upstate Cerebral Palsy can quickly access these programs for individuals and families that are in need of supports.

Upstate Cerebral Palsy currently serves nearly 500 residents in Oneida, Lewis, Herkimer and Madison Counties with MSC.  The department has an established leadership staff with over 77 years of total experience.  The MSC staff has provided consistency of service to families throughout the redesign of the program in 2010 and is prepared to meet the challenges and transition in the future of the 1115 Waiver project.  Our goal is to help, support and guide people with disabilities through professional, compassionate person centered care.
